    Study of histopathological pattern of endometrium in abnormal uterine bleeding in a tertiary care center

    Background: It is estimated that 9-30% of women of reproductive age suffer from abnormal uterine bleeding. As most cases are associated with anovulatory menstrual cycles, adolescent and perimenopausal women are particularly vulnerable to this particular condition. The aim of this study was to evaluate the histopathological pattern of endometrial biopsy from patients presenting with abnormal uterine bleeding. Methods: The retrospective study was conducted in the Department of Pathology in tertiary care centre from 1st January 2021 to 31 December 2022. All the patients presenting with abnormal uterine bleeding and who underwent endometrial biopsy were included in this study. Distribution of various histopathological patterns was observed in the different age groups. Results: A total of 260 cases were included in the study. The commonest pattern in these patients was proliferative endometrium (31 %) and products of conception (31%), hyperplasia without atypia (13.7%). Other patterns identified were secretory endometrium, chronic endometritis, endometrial polyp, complete mole, partial mole, and Ca endometrium. Conclusions: As observed from the study, there is an age specific association of endometrial bleeding, with highest incidence in 21-30 years of age group. Hence, dilatation and curettage are helpful for diagnosis, to assess therapeutic response and to know the pathological incidence of organic lesions in cases of abnormal uterine bleeding

    Glucosamine HCl-based solid dispersions to enhance the biopharmaceutical properties of acyclovir

    The objective of the work presented here was to assess the feasibility of using glucosamine HCl as a solid-dispersion (SD) carrier to enhance the biopharmaceutical properties of a BCS class III/IV drug, acyclovir (ACV). The solid-dispersions of acyclovir and glucosamine HCl were prepared by an ethanol-based solvent evaporation method. The prepared formulations characterized by photomicroscopy, scanning electron microscopy (SEM), differential scanning calorimetry (DSC), Fourier transforms infrared spectrophotometry (FTIR), powder x-ray diffractometry (PXRD) and drug content analysis. The functional characterization of ACV-SD was performed by aqueous solubility evaluation, dissolution studies, fasted versus fed state dissolution comparison, ex vivo permeability, and stability studies. Photomicroscopy and SEM analysis showed different surface morphologies for pure ACV, glucosamine HCl and ACV-SD. The physical-chemical characterization studies supported the formation of ACV-SD. A 12-fold enhancement in the aqueous solubility of ACV was observed in the prepared solid dispersions, compared to pure ACV. Results from in vitro dissolution demonstrated a significant increase in the rate and extent of ACV dissolution from the prepared ACV-SD formulations, compared to pure ACV. The rate and extent of ACV permeability across everted rat intestinal membrane were also found to be significantly increased in the ACV-SD formulations. Under fed conditions, the rate and extent of the in vitro dissolution of ACV from the formulation was appreciably greater compared to fasted conditions. Overall, the results from the study suggest the feasibility of utilizing glucosamine HCl as a solid dispersion carrier/excipient for enhancement of biopharmaceutical properties of acyclovir, and similar drugs with low solubility/permeability characteristics

    The Cosmology of Massless String Modes

    We consider the spacetime dynamics of a gas of closed strings in the context of General Relativity in a background of arbitrary spatial dimensions. Our motivation is primarily late time String Gas Cosmology, where such a spacetime picture has to emerge after the dilaton has stabilized. We find that after accounting for the thermodynamics of a gas of strings, only string modes which are massless at the self-dual radius are relevant, and that they lead to a dynamics which is qualitatively different from that induced by the modes usually considered in the literature. In the context of an ansatz with three large spatial dimensions and an arbitrary number of small extra dimensions, we obtain isotropic stabilization of these extra dimensions at the self-dual radius. This stabilization occurs for fixed dilaton, and is induced by the special string states we focus on. The three large dimensions undergo a regular Friedmann-Robertson-Walker expansion.     Role of sonography in the assessment of dengue fever with serological correlation

    Background: Dengue, caused by a flavivirus has emerged as a major public health problem in the Indian subcontinent. This study was to assess severity of disease by ultrasound findings and to correlate ultrasound findings with blood platelet count.Methods: A Cross sectional study carried out on 210 subjects who had serologically proven dengue fever were included in the study. Ultrasound of abdomen, pelvis and thorax was performed and imaging features were analysed by descriptive and analytical statistics using SPSS version 16. Chi-Square test used and P values ≤ 0.05 was considered to be statistically significant.Results: Mean age of patients 42± 19 years with 106(50.5%) males and 104(49.5%) females. The Gall bladder wall thickening was noted in majority (92.8%) of the study subjects followed by Right peri-renal collection in 112(53.3%), Splenomegaly in 105(50.0%) subjects and Hepatomegaly in 66(31.4%). The sonographic abnormalities including Gall bladder wall thickening, Right, Left and Bilateral perirenal collection, Splenomegaly, Hepatomegaly, Right and Left pleural effusion, were significantly higher in study subjects with decreased platelet count.Ascites was the commonest finding in 76 study subjects with platelet count between 60000-79000 (36.2%) and in 144 study subjects (68.5%) in the 20-59 years age group. As the   platelet count decreased the severity of ascites increased and was statistically significant (p≤0.05).Conclusions: A patient who presents with sonographically recognizable complications is more likely to have disease that requires immediate and aggressive management.

    Vitamin D deficiency and its correlation with pregnancy outcome

    Background: There is a growing concern about the high prevalence of vitamin D deficiency and its relationship with variety of diseases worldwide. The objective of this study was to determine the prevalence of vitamin D deficiency and its association with pregnancy outcome.Methods: This was a cross sectional study conducted among 150 antenatal women from October 2014 to April 2015. Data containing socio-demographic details, vitamin D level, serum calcium, pregnancy complications and growth situation of newborns were collected and analyzed.Results: A total 150 pregnant women participated in the study, 75.3% were vitamin D deficient, 22.1% vitamin D insufficient and 2.6% normal. There was no significant adverse maternal outcome. NICU admission was required in 28.3 and 15.2% in the vitamin D deficient and insufficient groups respectively.Conclusions: Women from different socioeconomic status, irrespective of parity and educational levels had vitamin D deficiency and insufficiency. This study fails to show a relation of vitamin D deficiency with other high-risk factors of pregnancy and does not show any adverse fetal outcome

    ROLE OF MATRA BASTI (ENEMA) OVER ABHYANGA (MASSAGE) AND SWEDA (SUDATION) IN REDUCING SPASTICITY IN CEREBRAL PALSY WITH SUDDHA BALA TAILA-A RANDOMIZED COMPARATIVE CLINICAL STUDY

    Objective of the study was to assess the efficacy of Matra basti (enema) over Abhyanga (massage) and Sweda (sudation) in reducing spasticity in cerebral palsy. Study was carried out in IPD, Department of Kaumarbhritya, KLEs Shri BMK Ayurved Mahavidyalaya, Shahapur, Belgaum, Karnataka; India.30 children fulfilling diagnostic criteria between the age group of 2-12 years were included and enrolled as per computer generated block randomization into 2 groups of 15 children in each group. Group A was administered with Matra basti (enema) after local Abhyanga (massage) with Suddha bala taila and local Swedana (sudation) with Nadi sweda (type of sudation) method for a duration of 15 days. In group B spastic children were treated with Sarvanga Abhyanga (massage all over body) followed by plain Nadi sweda with Suddha bala taila. Follow up was done on 30th and 45th day of treatment. Both the procedures were helpful in reducing spasticity. Matra basti (enema) is more effective in treating the spasticity of cerebral palsy as compared to Abhyanga (massage) and Sweda, whereas Abhyanga (massage) and Sweda (sudation) is effective in treating fine motor functions. Suddha Bala taila is effective in treating Ekanga vata (monoplegia), Pakshaghata (diplegia) and other related Vata Vyadhi (neurological disorders) as attributed to its Rogaghnata (disease). Thus the procedures are effecting in reducing spasticity
